HCS - Home and Community Based Services

 

Our participation in this Medicaid Waiver Program certifies us to offer services to state qualified folks in need of several different types of care, such as:

 

• Residential Support (RSS) - We have one 3 bed HCS home that is located on the NW side of San Antonio. The folks who live in this group home require on-going staff support and assistance.

• Supervised Living (SL) - The folks who live here requires staff to be present 24 hours but only in supportive capacity. These individuals live more independently than the (RSS).

• Our Independent Living Program is in an apartment complex that gives you easy access to friends and family. (is this where this information goes?)

• Supported Home Living (SHL)

• Host/Companion Care (HCC) - Our host care providers are close family and friends, they are some of the most dedicated, friendly, and experienced staff in the industry.

• Community First Choice (CFC PAS/HAB) - This service is for individuals who live independently in the community or who remain in the family’s residence. Staff provides assistance to the individual during most needed hours.

• Host/Companion Care (HC): A family member or companion may provide this service. The individual would reside in the foster care/companion’s home while receiving services.

 

 

 

 

ICF-ID - Intermediate Care Facility for Individuals with Intellectual Disability or Related Condition

 

This program certifies us to offer residential services to state qualified folks in need of 24 hour a day care and supervision.

 

We strive to give our folks a “home" where they feel safe and secure. A place where they can learn how to develop skills such as community integration, safe medication administration, and transportation planning. This helps them be their best and live as independent as possible. We’ve found that establishing an all male home helps these men excel at this goal. Our second home is co-ed where some of our folks have lived for over a decade. Both of our ICF-ID homes are located on San Antonio’s north west side in safe and friendly neighborhoods.
